The Florida Department of Health in Osceola County is preparing for the next allocation of COVID-19 vaccine. FDOH-Osceola staff has contacted those who registered by phone or email prior to registration closing to offer appointments.

Effective Monday, January 18, a new process will be used to request an appointment. A COVID-19 Vaccination Appointment Request Form may be submitted by those interested in receiving a first dose vaccination or a second dose if the first dose was not administered by FDOH-Osceola. The link to this form will be posted on the Florida Department of Health in Osceola County’s website at http://osceola.floridahealth.gov and on Osceola County’s website at https://osceola.org/covid19vaccinations. Vaccination appointments will be scheduled January, 21 through January 30 based on vaccine availability. Submission of the form does not guarantee an appointment. Appointment confirmations will be sent via email.

Regarding second dose vaccinations, if the first dose of vaccine was administered by FDOH-Osceola, staff will follow-up to schedule the second dose appointment. These individuals are asked to monitor their email and not submit the COVID-19 Vaccination Appointment Request Form. Those who received a first dose vaccination at FDOH-Osceola and have not been contacted 2 days prior to their second dose due date should call 407-552-0120, Monday through Friday from 8:00 a.m. to 5:00 p.m.

FDOH-Osceola vaccination events are relocating to Centro Cristiano Dios De Pactos Church located at 1130 Simmons Road, Kissimmee, FL 34744. Appointments are required for these events. To minimize traffic congestion and wait times, those with appointments should arrive at their scheduled time. Those arriving early will not be allowed to proceed through the line.
