Florida Department of Health to partner with Community Hope Center, other locations for WIC services and vital statistics

The Florida Department of Health in Osceola County is excited to announce the availability of select Vital Statistics and WIC services at the Community Hope Center located at 2198 Four Winds Blvd. Kissimmee, FL 34746.

On Wednesdays, FDOH-Osceola staff will be onsite to process WIC certifications, re-certifications, mid-certifications, and EBT issuance by appointment from 9:00 a.m. to noon and birth certificates on a walk-in basis from 1:00 p.m.-3:00 p.m.

“We are excited about this partnership with the Community Hope Center which expands access to FDOH-Osceola services. This not only benefits the clients of the Community Hope Center but anyone in need of the services offered.”

birth certificates for all years and death certificates from 2009 to current date.
• Osceola County Tax Collector’s Office, 2501 E. Irlo Bronson Memorial Hwy, Kissimmee, FL 34744 and 1300 9th
Street, Suite 101B St. Cloud, FL 34769, Office Hours: Monday-Friday 8:00 a.m. – 4:00 p.m., birth certificates only.
• FDOH-Osceola 105 N. Doverplum Ave, Kissimmee, FL 34758, Office Hours: Tuesday and Thursday 9:00 a.m. –
3:00 p.m., birth certificates for all years and death certificates from 2009 to current date.

Florida death certificate for a death occurring before 2009.
