The Central Florida Collaborative has launched the 2022 Community Health Needs Assessment and there are still a few days to help identify our region’s current needs and issues by completing the 2022 Survey.

This survey is conducted in partnership with Hospitals, County Health Departments, and Federally Qualified Health Centers in the Central Florida region. The survey gives our community’s residents the opportunity to provide feedback on the quality of life, access to care, food insecurity, transportation, housing, health needs for adults and children, and much more.

The data collected from this survey will have complied in regional and county-specific reports. This information is essential to developing our County’s Community Health Improvement Plan (CHIP) which is required by the Florida Department of Health. It is also utilized by many community-based organizations as they develop their strategic plans.
