The Florida Department of Health in Osceola County (FDOH-Osceola) is currently not accepting COVID-19 vaccination appointment request forms. Please continue to monitor their website at http://osceola.floridahealth.gov for updates on vaccine availability.

For those who have already requested appointments, if time and date are available, confirmation emails will be sent prior to the appointment time. Vaccination appointments will be scheduled from January 21 through January 30 based on vaccine availability. Submission of the form does not guarantee an appointment. Appointment confirmations will be sent via email, according to the FDOH in Osceola County.

Regarding second dose vaccinations, if the first dose of vaccine was administered by FDOH-Osceola, staff will follow-up to schedule the second dose appointment. These individuals are asked to monitor their email and not to submit the COVID-19 vaccination appointment request form. Those who received a first dose vaccination at FDOH-Osceola and have not been contacted two days prior to the second dose due date should make contact with their call center at 407-552-0120, Monday-Friday from 8:00 a.m. to 5:00 p.m.

FDOH-Osceola vaccination events are relocating to Centro Cristiano Dios De Pactos Church located at 1130 Simmons Road, Kissimmee, FL 34744. Appointments are required for these events. To minimize traffic congestion and wait times, those with appointments should arrive at their scheduled time. Those arriving early will not be allowed to proceed through the line.
